The subject of the order is a medicine with the active substance burosumab, known as Crysvita (hereinafter only the product), for pediatric patients with x-linked form of hypophosphatemic rickets, when all the criteria listed below are met:
1) with a documented high serum concentration of FGF-23
2) with genetically confirmed to himtation in the PHEx gene
3) persistence of low hypophosphatemia despite long-term substitution with maximum tolerated doses of phosphate solution and active form of vitamin D
4) in progression of bone deformities despite administration of maximum tolerated doses of standard treatment (phosphates and vitamin D).
