Description : The Health Enhancement to Reduce Opioid-Use-Disorder (HERO) Project will fund organizations with established access to the focus population to increase time for existing staff to provide care coordination for hepatitis C testing and cure while concurrently promoting referrals and linkages to treatment for opioid use disorder. Activities for the HERO care coordination project include: -Implement internal HERO care coordination process/workflow, in collaboration with the Hawai‘i Department of Health (HDOH);-Provide HERO care coordination, which includes, but is not limited to: 1.Referring individuals who access hepatitis C testing or treatment to care supporting medication for opioid use disorder (if appropriate); 2.screening clients on medication for opioid use disorder for hepatitis C antibody and RNA (if antibody positive) testing and providing appropriate follow-up (e.g., hepatitis C treatment; harm reduction services);3.other care coordination services, such as scheduling client appointments; coordinating care with treating providers; ensuring access to and reimbursement for medications; referring patient to other social services; documenting in medical records; -Evaluate HERO through data reports, which includes:-Providing HDOH with monthly data reports and upon request;-Providing summary of challenges and successes as well as policy recommendations;-Attend monthly HERO implementation meetings; -Attend monthly Hep Free 2030 implementation meetings;To maximiz...
