The subject of the contract is the delivery:

1) gloves, clothing, aprons, masks, personal protection caps (package No. 1-8),

2) vascular catheters at the vascular surgery department(Package No. 9)

3) Surgical implants to the neurosurgery department (package 10),

4) orthopedic implants to the trauma and orthopedic ward (package 11-16)

for the needs of the Provincial Specialist Hospital SP ZOZ im. Stefan Cardinal Wyszyński in Lublin with the possibility of submitting partial offers- as specified in the assortment and price form, constituting Annex 3 to SWZ.

1. The subject of the order is: delivery of gloves to x-ray, according to the 1st assortment item-as specified in the assortment and price form constituting Annex 3 toSwz.

2. The subject of the contract offered by the Contractor must have features in accordance with the description contained in Annex 3 to SWZ - the assortment and price form and meet the conditions set out in this SWZ.

3. Medical devices offered must be approved for trading and use on the premisesPolish in accordance with applicable law and have documents confirming the relevant procedure for assessing compliance with the requirements set out in the Act of April 7, 2022r....
