1. The subject of the order is the supply of various medicines, including cytostatic and program medicines for the needs of the Provincial Hospital. John Paul II in Bełchatow.
2. Code and name according to the Common Procurement Vocabulary (CPV):
33.60.00.00-6 Pharmaceutical products
3. Description, size and scope of the subject of the orderia:
1) The ordering party divided the subject of the order into 10 parts (Packages No. 1-10), in accordance with the description included in Chapter xIII of the SWZ.
2) The remaining requirements regarding the implementation of the order and the principles of cooperation between the ordering party and the contractor are specified in the draft provisions of the contract regarding Withpublic procurement, constituting Annex No. 3 to the SWZ.
3) If the description of the subject of the contract includes indications of trademarks, patents or origin, source or a specific process that characterizes products delivered by a specific contractor, it should be assumed that the indication twhich is accompanied by the words "or equivalent". In such a case, the contracting authority allows the contractor to submit an offer for an equivalent preparation, and such an offer consists in offering preparations identical to the subject of the contract, for which there is a common name according to the index of names betweenancestral. If the ordering party has any doubts regarding the equivalence of the offered drugs, the contractor will be obliged to provide explanations and demonstrate their equivalence at each request of the ordering party. It is not allowed to submit offers for preparations registered as "dietary supplements".
