Quotation are invited for Dental Treatment Service.

SECTION B - SCOPE OF WORK
The dental treatment is estimated to be completed with approximately 30 working days. Meaning
that, the dentist dental staffs need 5 days preparing the medical materials, logistic requirement including
mobilizing the dental team, 10 days for conducting level 1 in Anchum Primary school and another 10 days for L2
dental treatment in Angseymar primary school and 5 days for reporting. The dentist dental staffs will go to the
respect schools to work with school directors and school health teachers and provide level 1&2 dental treatment.
• Prepared all necessary materials and instruments and ensured there are adequate team
members with expert to deploy at the field to perform the tasks as described in the objectives.
• Conducted LI dental treatment to 597 students of Angchum primary school and L2 dental treatment to 500
students of Angseymar primary school.
• Report the number of students, who need urgent referral for further dental treatment.
• Develop and submit the completion report of level 2 dental
treatment with annexes of data of students who got treatment (annex the format of completion
report and summary data of students, who level 2 dental treatment.
After the dental treatment, the lead technical
team will coordinate to consolidate the results as report and summit to CRS team. The
report should comprise of data of students who got level 1&2 dental treatment and students who need urgent
referral and level 2&3 treatment. During the report, it will be also actively involvement of CRS
Project Officer to provide feedback on the drafted report.

Submission of Quotations: Quotations must be submitted to the address provided in the contact details, no later
than the date and time of the deadline.
Submission of offers. Submit signed and dated offers to the office specified in this solicitation at or
before the exact time specified in this solicitation. At a minimum, offers must show:
• The solicitation number;
• The time specified in the solicitation for receipt of offers;
• The name, address, email, and telephone number of the Bidder;
• Terms of any express warranty;
• Price and any discount terms;
• "Remit to" address, if different than mailing address;
• A completed copy of the certifications and registration with the Dental Council of Cambodia;
• Acknowledgment of Solicitation Amendments;
• Past performance information, when included as an evaluation factor, to include recent and relevant
contracts for the same or similar items and other references (including contract numbers,
points of contact with telephone numbers and other relevant information).
