Description : The Cheshire and Merseyside ICS (Integrated Care System), through its Regional Care Home Support for Care Home steering Group, is seeking to support its local place leads and Care sector providers with the adoption of digitally enabled healthcare tools. We are completing an initial scoping exercise and would particularly like to facilitate an open discussion with suitable providers of Digital Health tools and systems who are currently supplying, or have in development, a Digital falls assessment tool that could be used within Care homes across the regional footprint. Improving care by reducing the incidences of falls is a key objective within the strategic document, Framework for Enhanced health in Care homes.
