Description : The Hawaii State Department of Health (DOH), Family Health Services Division (FHSD) seeks to enter a contract with a qualified, in-state organization to help facilitate planning and implementation of the Title V Maternal and Child Health State Action Planning Phase which is the next step of a five-year maternal and child health (MCH) needs assessment mandated for the federal Title V MCH Block Grant.FHSD utilizes the federal Title V MCH Block Grant (‘Title V-) as a framework to promote the development and coordination of systems of care for the MCH population and improve population health outcomes.To ensure that the MCH population health needs are met, the Title V grant requires a state-level needs assessment every 5-years to collect and analyze data, identify health priorities, develop a state action plan, resource the plan, and disseminate information. The prioritization phase is currently being completed. The next phase of the needs assessment is to develop the state action plan, which includes determining action strategies for each priority need, and identification of outcomes, performance measures, and data sources that demonstrate the state-s progress in addressing the needs and achieving the target outcomes. The priorities will span six population domains, and each domain will have a team of participants.
