INVITATION TO BID FOR

PROCUREMENT OF BLOOD GLUCOSE, CHOLESTEROL & URIC ACID METER 3-IN-1 KITS

IB NO. 24-32

1. The Department of Health-Bicol Center for Health Development (DOH-Bicol CHD), through the General Appropriations Act (GAA) intends to apply the sum of One Million Fifty Thousand Pesos Only (Php 1, 050, 000.00) for the Procurement of Blood Glucose, Cholesterol & Uric Acid Meter 3-in-1 Kits under IB No. 24-32. Bids received in excess of the ABC shall be automatically rejected at bid opening.

2. The DOH-Bicol CHD now invites bids for the above Procurement Project. Delivery of the Goods and Services is required on the expected delivery date (s) stated in Section VI, Schedule of Requirements. Bidders should have completed, within three (3) years from the date of submission and receipt of bids, a contract similar to the Project. The description of an eligible bidder is contained in the Bidding Documents, particularly, in Section II (Instructions to Bidders).
