Description : NHS South West London CCG on behalf of all London CCGs is lead commissioner for a London Dynamic Integrated (Urgent) Care Plan (UCP). The UCP is predominantly a software service that enables the sharing with urgent care services (111, London Ambulance Service, Emergency Departments in Acute Trusts) of treatment escalation plans, crisis care plans, end of life care plans, patient specific protocols and personalised care plans that have been co-developed and documented by different health and care disciplines.On behalf of London CCGs, as the lead commissioner of the service, South West London (SWL) CCG, is seeking an innovative, collaborative development partner or supplier to build on the existing and emerging care planning and digital initiatives to transform the way care plans are captured and made available to patients and the health and social care system.
