TIN Certificate with Update Tax Payer Certificate;4. General experience of Supply (According to TDS);5. Relevant/Similar & Specific Experience of Supply (According to TDS), 5. Sufficient liquid assets/financial solvency Certificate/credit line facilities (use CPTU prescribed format) and7. Manufacturer's Authorization Certificate.8. The Responsive Tenderer Will Provide Service Regarding to Repair and Maintenance of all instruments/Equipment including IT Support for five years.9. Those who have previously worked in the Simulation Laboratory at ME&HMD, Supply and Installation of Medical Equipment, Supply and Installation of MSR under 4th HPNSP, they are requested to submit a Contact completion certificate. Otherwise, they will be considered as non-responsive. Hiding of any previous works regarding above mentioned issues will be also considered as non-responsive.
