Declaration of Primary Current Injection Test Set Manufacturer Authorized Supplier service Centre address and company contact person.08. Origin and brand of components mentioned in the specification.09. Test Report from any reputed agency (Third Party Test Report) during machine supply.10. Nominated Engineer Bio-Data and declaration of assignment for this project (Declaration with company Pad by Owner /Authorized Person).11. Factory Test Report during machine supply.12. Similar type experience must be needed of minimum Tk in lakh 5.00 [Five Lakh] under maximum 3 [three] contracts within last 05 [five] years. (Attached with Purchase Order and Work Completion Certificate).13. General Experience must be needed of minimum Tk in lakh 7.00 [Seven Lakh] under maximum 5 [Five] contracts within last 05 [five] years. (Attached with Purchase Order and Work Completion Certificate).14. Credit Commitment of Line of Credit needed Tk in lakh 6.00 [Six Lakh] from date of tender publication. If the Supplier submitted credit commitment, the commitment should be without any alteration or correction or modification of the form of “e-PG3-8”.15. A properly filled technical specifications and compliance sheet for the offered machine must be supplied with the offer as part of the tender.
