The Joint Medicines Policy spans across Lewisham Adult Social Care and NHS South East London ICB (NHS SEL ICB) - Lewisham, in collaboration with University Hospital Lewisham (UHL)/ Lewisham & Greenwich NHS Trust (LGT), Lewisham Integrated Medicines Optimisation Service (LIMOS), the Local Medical Committee (LMC) and Community Pharmacy South East London (CPSEL)It provides a common policy and procedure and a single agreed assessment pathway for the provision of pharmaceutical and medicines support across the NHS and Social Care in Lewisham borough.
