City and County of San Francisco Human Services Agency (HSA) Funding Opportunity Request for Proposals (RFP) #1174 Health Promotion Programs 1. Older Americans Act Title III D - Disease Prevention and Health Promotion Program for Older Adults And 2. Health Promotion Program Older Adults and Adults with Disabilities The San Francisco Human Services Agency (SFHSA) and the Department of Disability and Aging Services (DAS) announces their intent to seek proposals from nonprofit organizations to provide health promotion programs for older adults and adults with disabilities. These programs are designed to promote physical health, prevent illness, and manage chronic health conditions. SFHSA and DAS are seeking proposals for two types of health promotion programs: 1. Older Americans Act Title III-D Disease Prevention and Health Promotion Program for Older Adults: The Older American Act Title III-D health promotion programs are evidence-based and have been shown to improve health and well-being and reduce disease and injury. Evidence-based programs are based on scientific evidence and have been demonstrated through rigorous evaluation to be effective in improving the health of older adults. Title III-D health promotion programs include those related to the prevention and mitigation of the effects of chronic health conditions such as hypertension, arthritis, heart disease, diabetes, obesity, arthritis, and osteoporosis. Title III-D health promotion programs can also focus on chronic pain management, stress management, fall prevention, physical activity, and support for caregivers. Health promotion programs are provided in a variety of settings including senior centers, community centers, and community dining sites.
