2.5 Philgeps Accreditation (Platinum Membership) valid until Dec. 31, 2025

2.6 Omnibus Sworn Statement duly notarized for CY 2025

3. Submit picture showing appearance of the required items and/or brochures

indicating the specifications of each items.
