SUBJECT: Simplified Acquisition Proposal Request (SAPR), FA445224R0018, Single Source - Brand Name Only (BNO) SimX Virtual Reality (VR)
This is a SAPR for the subject requirement to be awarded pursuant to Federal Acquisition Regulation (FAR) FAR Part 13 ordering procedures, FAR Part 12 procedures, and the specific requirements outlined in this request with attachments. Headquarters Air Mobility Command (AMC) Surgeon Generals Medical Modernization Branch (SGR) requires the purchase of SimX VR Training Kits. It is the intent of the 763d Enterprise Sourcing Squadron to award a single source-brand name only, firm-fixed price (FFP) purchase order to SimX, Inc. located at 548 Market Street #87927, San Francisco, CA 94104, CAGE 79S56. The awardee shall deliver the BNO SimX VR products in accordance with the delivery dates outlined in Attachment 1. In response to the SAPR, the contractor shall complete and return the following: - Information and Pricing Worksheet (Attachment 1), or a submitted proposal in offeror format as long as it, at a minimum, depicts unit prices, extended prices, and a total (rounded to the nearest dollar), provides applicable specifications, and submits a response providing the information being requested in paragraphs 1 - 11 - Provisions and Clauses (Attachment 2), the contractor shall complete the fill-in provisions and clauses outlined in Attachment 2. - Redacted Single Source Justification (Attachment 3) (Will be provided/posted to SAM.gov by the Contracting Officer and/or Contract Specialist) - The contractor shall include a cover letter stating a proposal acceptance period of at least 30 days.
