Description : Solent NHS Trust, invite interested parties to tender for a contract to provide sponsorship for the continuation and future development of a Southampton based Stoma Service. The aim of the Stoma Care Service is to provide seamless care throughout the patients' pathway from pre-operative referral, inpatient stay and continued management within the community. This means that the service have structured their provision of care to support all aspects of the patient pathway, targeting the specific needs at each point and ensuring interventions are in the right place at the right time. The stoma care service will provide informal counselling, education, lifestyle advice, health promotion, diagnosis of stoma complications and treatment, appliance management including reviews and assessment of usage and support to patients who have or are about to have a stoma formed.
