PROJECT: SUPPLY AND DELIVERY OF QUADRIVALENT SEASONAL INFLUENZA VACCINE WITH ONE (1) DAY VACCINE ADMINISTRATION
LOCATION: PCA CENTRAL OFFICE, COVERED COURT, DILIMAN, QUEZON CITY
I. SCOPE OF WORK
Supply and delivery of 109 dose of quadrivalent seasonal influenza vaccine with the following details:
- 0.5mL prefilled syringe
- Suspension for injection (IM/SC)
- 2025 Southern Hemisphere
- WHO Recommended strains
One (1) Day Vaccine Administration with the following details:
- One (1) Medical Doctor / Registered Nurse for One (1) day ONLY for the vaccine administration.
- Provides vaccine administration supplies such as alcohol pads, plaster strips, sharps collector, consent forms, vaccination cards
- All medical team are wearing proper attire during the administration of the vaccines
- Disposal of used materials in vaccination
