SCOPE OF WORKS:

WORK ENVIRONMENT MEASUREMENT FOR THE FOLLOWING:

PARAMETERS NO OF SAMPLING AREA/STATION

1. ILLUMINATION 15

2. NOISE 15

3. VENTILATION 15

4. HEAT STRESS 15

5. RELATIVE HUMIDITY AND AIR 15

6. TOTAL RESPIRATORY DUST (TRD) 15

1. SAMPLING SHOULD BE CONSISTENT WITH DOLE STANDARDS. ALL SAMPLES SHALL BE PROPERLY LABELED

WITH SPECIFIC PROJECT CODE AND NUMBER TO MAINTAIN THE OBJECTIVITY OF THE ANALYSIS.

LABORATORY CERTIFICATE ANALYSIS SHOULD BE ATTACHED ON THE MONITORING REPORT.

2. MONITORING REPORT TO BE SUBMITTED WITHIN 30 WORKING DAYS AFTER COMPLETION OF TEST.

3. SHOULD BE ACCREDITED WEM PROVIDER BY DOLE.

4. THE WINNING BIDDER TO COORDINATE WITH SAFETY OFFICERS REGARDING THE SCHEDULE AND

SAMPLING POINTS.

5. THE WINNING BIDDER MUST PROVIDE AT LEAST ONE (1) SAFETY OFFICER 2 (SO2). THE SAFETY OFFICER

IS RESPONSIBLE FOR MONITORING AND ASSESSING HAZARDOUS AND UNSAFE SITUATIONS AND PROVIDING

SAFETY MEASURES TO ASSURE PERSONNEL AND PRORPERTY SAFETY THROUGHOUT DURATION OF THE

ACTIVITY.
